The extra costs an aplastic anaemia diagnosis brings can be overwhelming for some. The welfare benefits system is designed to meet the needs of patients in these difficult times. Not all benefits are income related; some depend on national insurance contributions and some on how the illness is affecting someone in their day-to-day activities.

The law says we must get your consent to do this. The information you put into this form is strictly confidential and will be held securely by The Aplastic Anaemia Trust. (Sensitive personal data is defined as information relating to any of the following: racial or ethnic origin, political opinions, religious beliefs, trade union membership, health and sexuality or sex life, offences and/or other convictions. )

For the purposes of the Act the Data Controller is:  Aplastic Anaemia Trust. We will never sign you up to receive any additional contact or pass on your data to any other organisation without your specific permission. You can read our privacy policy at theaat.org.uk/privacy-policy

We will share the information you have put into this form with our contacts at Maggie’s, for the purposes of organising your consultation. We will not share any other information about you, and your information will be kept private and confidential by our partner charity.

By checking the box below you consent to us keeping a record of your case and, in line with the  General  Data  Protection  Regulation and Aplastic Anaemia Trust Privacy Notice, that the Trust can destroy your file after 6 years. You can access the data, ask for it to be changed, request for it to be erased, request restriction on use, or challenge any automated decision process at any time.
